NNS is seeking a Sleep / ENT Nurse Navigator to support, educate, assess, and guide patients through their care journey related to obstructive sleep apnea (OSA) and related airway conditions. This role focuses on patients evaluating or preparing for advanced therapies, including surgical or implantable options, especially those who have struggled with CPAP therapy.
Nurse Navigators communicate with patients through inbound and outbound telephonic outreach, video platforms, and SMS. Using empathy, motivational interviewing, barrier management, and clinical expertise, the goal is to improve patient understanding, optimize candidacy progression, and ensure appropriate movement through the diagnostic and treatment pathway.
Job Responsibilities
- Navigate patients through EMR and analytic platforms (where applicable).
- Conduct scheduled and triggered outreach via phone, video, and SMS in alignment with clinical pathway guidelines.
- Establish trusting, supportive relationships with patients managing sleep-disordered breathing or airway conditions.
- Educate patients on sleep apnea, diagnostic testing (home sleep studies, in-lab studies), CPAP therapy, and advanced treatment options.
- Assess treatment tolerance, CPAP adherence, and symptom improvement.
- Identify patients who may be candidates for advanced therapy consults and schedule appropriately per workflow.
- Communicate with providers regarding patient barriers, non-adherence, symptom persistence, or candidacy updates.
- Support coordination of diagnostic steps (sleep studies, imaging, DISE procedures, surgical consults).
- Utilize CRM and telephony systems to track patient progress and trigger appropriate follow-up activities.
- Ensure patients understand next steps, insurance requirements, and pre-procedure expectations.
- Maintain accurate and timely documentation in EMR and analytic software.
- Escalate concerns, compliance issues, or pathway breakdowns to Navigation Manager and/or Champion Providers.
- Maintain strict HIPAA compliance and confidentiality of patient data.
Clinical Training & Qualifications
- LPN or RN licensure required and in good standing.
- 5+ years of clinical experience.
- Compact multi-state licensure preferred.
- Experience in Sleep Medicine, ENT, Pulmonology, or Surgical Coordination preferred.
- Familiarity with obstructive sleep apnea management and CPAP therapy strongly preferred.
- Experience coordinating advanced therapies or surgical pathways is a plus.
- Strong IT proficiency with EMR systems, CRM platforms, and telehealth tools.
- Excellent verbal and written communication skills.
- Highly organized with strong time management and prioritization skills.

This is a part-time remote role. Hourly rate for this position is $29-$33.
